INSTRUCTION NO. ________

On retiring to the jury room, select one of your number as a foreman, who will preside over your deliberations. Appropriate forms of verdict will be submitted to you with any instructions. Use only the one(s) conforming to your conclusions and return the other(s) unused. [In this case there are (number) possible verdicts (as to each count [or] as to Counts number....). Only one of the possible verdicts may be returned to you (as to any particular count).]

A verdict may be reached by three-fourths of your number, or nine of you. As soon as nine or more of you shall have agreed upon a verdict, you should fill it out, if necessary, and have it signed. If your verdict is unanimous, your foreman alone will sign it; but if nine or more, but less than the entire jury, agree, then those so agreeing will sign the verdict.

As soon as you have completed and signed the verdict(s), you will notify the bailiff, who will then return you into open court.
